Home Health Care

This service provides medical and health-related services to support day-to-day activities to people in their homes. We offer these supportive services to maintain the safety and security of our clients at home. Accra’s home health care options include professional nursing, home health aide, homemaker, and rehabilitation services. Accra offers these services in the Twin Cities metro area and select counties throughout the state.

Accra primarily provides Home Health Care services to people requiring continuing care for chronic needs. These services are also sometimes offered on a short-term basis to people moving from a hospital or rehabilitation center back into their home.

Professional Nursing Services assist clients with a variety of in-home healthcare needs, including:

  • Health assessments and monitoring
  • Ongoing communication with physicians, pharmacists, Home Health Aides and family
  • Post-hospital and post-surgical care
  • Medication management and administration
  • Injection, blood draws and lab collections
  • Monitoring blood pressure, pulse and respiration
  • Ordering supplies and equipment
  • Wound care
  • Foot care
  • Ostomy care
  • Incontinence, catheter and colostomy care
  • Assistance with therapeutic and special diets
  • Diabetic management including monitoring blood sugars
  • Supervision of Home Health Aides

Accra’s Home Health Aide Services support the independence of people living at home by helping them with day-to-day activities, such as:

  • Bathing
  • Dressing
  • Oral hygiene
  • Skin care
  • Ambulation and transfers
  • Medication assistance
  • Exercise
  • Meal preparation 
  • Escorts to meals, social, and educational activities
  • Arranging transportation and appointments 
  • Safety checks

Homemakers assist clients with household activities when the primary caregiver or family member who typically provides these services is unable to manage the activities or temporarily absent. Homemakers may monitor the person’s well-being while in the home, including home safety.

Homemaker services are provided by a trained professional and can include:

  • Light cleaning, sweeping, mopping, vacuuming and dusting
  • Washing, drying and putting away dishes
  • Emptying the trash
  • Laundry
  • Changing linens and making beds
  • Meal preparation
  • Shopping for food, clothing and supplies
  • Helping with personal errands
  • Transportation arrangement
  • Routine household care
  • Watering and caring for household plants
  • Social stimulation

Accra offers rehabilitation services that support independence by helping people restore skills and functions for daily living. We send our dedicated team of therapists to the home or living facility to provide traditional therapeutic techniques. Therapists will carefully assess the clients’ needs and then provide the appropriate service with expertise and care.

Rehabilitation services from Accra are available in select locations and include:

  • Physical Therapy
  • Occupational Therapy
  • Speech-Language Services

After the initial call, Accra may offer a free home evaluation to determine if your care needs can be met at home. We will also:

  • Check insurance eligibility to confirm services are covered by your insurance
  • Obtain home health orders from your primary care physician or assist you in finding a primary care physician
  • Provide resources for services you need that Accra does not offer
